button "Add method and create new" for some enrolment methods

      1. Create several cohorts (not necessary to have members in them)
      2. Enable meta enrolment method and make sure you have several courses in the system
      3. Create course, go to enrolment methods page
      4. Add a cohort enrolment method, press "Add method and create new", add another, press "Add method". Make sure workflow is working, methods are added
      5. Edit a cohort enrolment method, make sure the new button does not appear and editing works as expected
      6. Add a meta enrolment method, press "Add method and create new", add another, press "Add method". Make sure workflow is working, methods are added

      I'm creating this as a new issue because it only represents a partial solution for issues MDL-49417 and MDL-27628

      Some enrolment methods such as meta and cohorts need a faster way to add multiple instances. As a quick solution I can suggest additional button "Add method and create new" on the new enrolment method form.
