Cannot enter an icon URL for a named external tool

      Description

      The fields for entering an icon URL are disabled when a named tool is selected. This prevents the icon specified in an LTI 2 tool proxy from being overridden, but also means that an icon URL cannot be entered for an LTI 1 tool.

      The proposal I am making here is that an icon URL should be added to the tool configuration page used by administrators for LTI 1 tools. If no icon URL is specified (for either LTI 1 or LTI 2 tools), then an instructor should be able to enter one when adding a link to a course.
