      1. As a teacher Create a database activity with a picture field
      2. As a student navigate to the database activity.
      3. As a student click on "Add entry"
      4. As a student Set the Title to "Moodle" and upload a picture

      Found while testing MDLQA-7889

      When adding a record to the database activity module and then clicking on the "Save and add another" button, The form that loads is populated with the previously saved data.

      Steps to reproduce:

      1. As a teacher Create a database activity with a picture field
      2. As a student navigate to the database activity.
      3. As a student click on "Add entry"
      4. As a student Set the Title to "Moodle" and upload a picture
      5. Click "Save and add another"

      Expected result: You will get a blank form to add a new entry
      Actual result: The previously added record is displayed including the uploaded pictures.
