      Description

      This issue is to optionally enable replacing missing answers with fake answers so a standard error message is shown and student is given the fake grade (default is 0).

      Previously, quiz attempts that had one of the answers that were removed would see a 'Coding error detected' for the Multi Choice question affected.

      Once enabled, the missing answer will have the text 'This answer choice has been removed (do not select)' to indicate this option isn't valid any more and new attempts shouldn't pick it.
