Accessibility enhancements: Indicate phase name/form name on the Workshop title

      1. Create a course
      2. Add workshop activity to a course
      3. Current phase with heading H3 display under workshop name
      4. Click My submission menu on the left, display My submission under workshop name
      4. In userplan table , for current phase , screen reader will read phase title and "currrent phase"

      As discussed at https://moodle.org/mod/forum/discuss.php?d=283014, the Workshop title is the same for all phases and forms which can cause confusion for screen reader user. It would be more accessible if the workshop title indicates which phase or which form the user is currently on.

      • view form: Display current phase name (<h3>) below the workshop title
      • submission form: display subheading of 'Submission' below the workshop title
