Overriding an image is not always possible in an HTML editor

      Description

      To reproduce:

      1. Create a new workshop activity with:
        • Submissions settings > Max attachments: 1
        • Example submissions enabled
      2. Set-up the phase 1
      3. Add a new example submission
        • Pick an image from wikimedia in the editor
        • Save
      4. Go back to edit the example submission
        • Double click the image and browse wikimedia for another image (it must have a different name to the first one)
        • Save

      Expected

      • The new image is displayed when viewing the submission

      Actual

      • The image is not displayed

      My personal opinion is to remove the limitations on how many files can be uploaded in an area. Or, if there is a limit to try to smart guess which ones are used. Though note that in some websites (MUA I think), a text editor area was abused to store files used in other locations, and it'd be better not to break that.
