undefined variable $OUTPUT in badges/criteria/award_criteria_course.php

      1. Login as admin.
      2. Turn debugging ON.
      3. Create a course and enable completion tracking.
      4. Under Course administration go to Badges ► Manage badges
      5. Add a new badge.
      6. On the badge's page, click on the Criteria.
      7. Under Add badge criteria, select Course completion
      8. Enter a value for the Minimum grade required, then click Save.
      9. Open another window and delete the course.
      10. Refresh the page with the badge settings.
      11. You should see exception "invalidrecordunknown"
