Image icon missing from user_bulk_display

When using the 'Display on page' option in bulk user actions, there is a missing image icon next to the 'Full name' field.

This issue does also appear in earlier versions (I've looked as far back as 2.6.11).

Steps to replicate

1. Go to Site administration / ► Users / ► Accounts / ► Bulk user actions
2. Select a user
3. In the 'With selected users... ' drop down choose 'Display on page'
4. Click 'Go'

A missing image icon should be displayed (see Capture.JPG)
