Multiple styling issues with Lti provider page

The page has following issues

1/ Icons are not aligned, see screenshot.
2/ The input fields do not have same look as other input fields in moodle. "Form control" class may be ?
3/ Should the "Add" button be on left/center or right?
4/ inline css is used in templates.
