Remove "publish" course option from manager-role

Hello,

You should remove "publish" option from course edit-menu from manager-role if hubs aren't registered, there is no need to show this option in the menu when all it does is give an error message;

"Publish on
Your administrator needs to register this site with at least one hub before you can publish a course. Contact your site administrator."
