Files submitted through the Assignment module are not being rendered when the Online text option is enabled. Instead a blank page is rendered with no pagination to navigation to the submitted file within the annotation interface. This happens regardless if a student submission includes content in the online text field.

Turning off the online text option for the assignment allows the submitted file to be rendered as expected.

Tested with our 3.3.1+ version and on demo.moodle.net
