Add tags icon for clean theme

      Hide
      1. Log in as admin
      2. Set clean theme (Site administration/Appearance/Themes/Theme selector)
      3. Go to a course.
      4. On the cog, choose More...
      5. Click on Question Bank link.
      6. Click on Create a new question... button and add a new question of any type, essay is fine.
      7. On the question bank list, make sure the tag icon is displayed correctly.

      Description

      During testing of MDL-61133, Helen Foster noticed the tags icon is missing on clean theme.

      That only affects clean, because on boost we have mapped font awesome tags icon.

      I will kindly request Barbara Ramiro help on this one.
