Step to reproduce:

Use Browser IE 10 on Win10 Or Firefox Or Chrome

Login as a student

Go to Course home page

Open any editor has plugin upload image.

Upload 1 image then save the current post.

Edit current post then upload file another image (different content) with the same name and format.

Then click button Overwrite.

Save image then observe the editor.

Actual: Add one more the uploaded image

Expected: The Preview thumbnail of the uploaded image will be changed to the new image

Please note that If you submit the form, it will still save the 2 files.

When overwrite new image with the same name with last uploaded image, file uploader not change preview to new image
