When using the plugin and recording multiple attempts, e.g. 3 attempts to record the video, the plugin will insert 3 videos, but all videos are of the last attempt.  It should only include the last video.

1. Click the record video button
2. Start recording
3. Stop recording
4. Click record again
5. Repeat steps 3 & 4 a couple of times
6. Click attach recording

Not tested with audio recording, but that could have the same issue
