Accessibility issues with color indications

      1. Login into a course.
      2. Open the user menu
      3. Hover over the icons before the links in the user menu and verify no "hover text" is shown for the icons.
      4. Open the course menu
      5. Hover over the icons before the links in the course menu and verify no "hover text" is shown for the icons.
      6. Create a "Missing words" question- any text will do - here is an example:

        Question text : Carrots are [[1]]
        Choice 1: Good
        Choice 2: Bad
        Choice 3: Sometimes OK
        

      7. Preview the question and answer it (Submit and finish). 
      8. Hover the tick or cross next to the answer and verify you Do see hover text showing "Correct" or "Incorrect".
      9. Inspect this icon in the browser and verify it does not have the attribute: aria-hidden="true"

    Description

      The issue appears in Corrected answers in quizzes: the description that the answers in one question are partially correct is read out. However, the red cross and the ggreen v that indicate the incorrect or correct answer are not read out. Nevertheless, the correct ansers that are displayed below as text are read out. The screen reader user needs to remember which answers were selected and compare them to the text that indicates the correct ansers (work around).
      Please provide the text equivalent for the information that are indicated in color (as it is not recommended to use only colors to convey information WCAG 2.0 SC 1.4.1).

      I used Mozilla firefox 52.5.0 (32 bit) with JAWS For Windows 2018 (build 1712.10 and NVDA 2017.4 as screen readers.
