Testing Instructions:
1. Log in as admin
2. Create a course
3. Add a feedback activity
4. Add one of each question type
5. Press Preview questions
6. Confirm each form element is locked/disabled

The feedback preview button next to the activity name may confuse students if they click it instead of using the "Answer the questions..." link.

It would be great to display some text for explaining this is a preview or to lock the feedback questions when accessing to the preview page.
