Bullet points and numbers shown in book table of contents (Boost only)

      Please test this patch on both themes Classic and Boost

      1. Login as admin
      2. Create a new course C1
      3. Add a book resource "Test Book" to course C1
      4. Create a first chapter called "First Chapter" with some dummy text
      5. After creating the first chapter you should be able to see your newly created chapter and the Table of Contents block
      6. From the Table of Contents block click the + icon after "First Chapter"
      7. Create a second chapter called "Second Chapter" with some dummy text
      8. Click the plus icon after "Second Chapter"
      9. Create a third chapter called "Third Chapter with a very long title" and check the subchapter checkbox.
      10. verify the chapter titles are numbered in the Table of Contents block
      11. Click the gear icon right of the book title "Test Book"
      12. On the dropdown choose Edit settings
      13. Open the "Appearance" Section and change Chapter formatting from Numbers to Bullets
      14. verify the chapter titles now have a bullet instead of a number
      15. verify the chapter titles are truncated when the title is too long
      16. verify the action icons are in the same row as the chapter titles
      17. Edit the Book settings
      18. Change Chapter formatting from Bullets to Indented
      19. verify no numbers show, the subchapter is still indented
      20. Edit the Book settings
      21. Change Chapter formatting from Bullets to None
      22. verify the is no formatting of the chapters

      Steps to reproduce (using Boost theme):

      1. Create a book with chapter formatting set as Numbers
      2. Add a couple of chapters
      3. View the book

      Expected result:

      The chapters are listed in the table of contents with numbers and no bullet points (as when using Clean theme).

      Actual result:

      Bullet points AND numbers are shown.
