Allow LTI in Course Navigation

      Prerequisite

      This needs to be tested with both LTI 1.1 and 1.3 tools. A good reference for test tools to use are in the testing instructions' prerequisite for https://tracker.moodle.org/browse/MDL-65306

      Usage

      1. Log in as an administrator
      2. Navigate to Site Admin > Plugins > External Tool > Manage Tools 
      3. Edit an existing tool
      4. Two fields have been added, "As menu link", and "Menu link label/URL"
      5. When "As menu link" is selected, this will allow the tool to be displayed in a list to choose from when adding a tool to the nav from a course.
      6. Multiple Menu link label/URLs are possible (depending on the nature of the lti tool). This allows multiple course nav items from the same tool to be added that serve different functions/views.
        1. You can click on "add menu link" to add as many as you want. 
        2. If no menu link label/url is added, it will use the Tool URL
      7. Once you have selected "As menu link" and added menu items, as an administrator or instructor, navigate to a course,  turn editing on in a course, and you will see  "course apps" appear in the left hand nav.
      8. Click on "Course apps" and you will then see the options available to choose from to add to your nav.
      9. Check the items you want to add to the nav and save
      10. The LTI menu items will now appear on the left under "course apps".
      11. Click on each LTI menu item to confirm it works as expected

      Course navigation - Add LTI tool launches to navigation area below "show all" above auto generated activity links. Clicking on launch brings up tool in main content area as an iframe
