Missing page title when prompting for upgrade key

      1. Set $CFG->upgradekey = 'something'; in your site config as per docs
      2. Bump any plugin version to trigger an upgrade
      3. Confirm that when prompted for the upgrade key, the page has an appropriate title ("Upgrade key required")
