Quiz add questions from question bank: problem with paging & show all

      1. Create a course.
      2. In the question bank create more than 20 questions (or, find a course that already has that many).
      3. Add a quiz to the course.
      4. Go to the Edit quiz page.
      5. Do Add -> From question bank ...
      6. Click >> in the paging bar, to go to page 2.
      7. At the bottom, click 'Show all N'
      8. Verify that all the questions are shown.
      Show
      Create a course. In the question bank create more than 20 questions (or, find a course that already has that many). Add a quiz to the course. Go to the Edit quiz page. Do Add -> From question bank ... Click >> in the paging bar, to go to page 2. At the bottom, click 'Show all N ' Verify that all the questions are shown.

      Steps to reproduce:

      1. Find or create a course with more than 20 questions.
      2. Create a new quiz.
      3. Go to the Edit quiz page.
      4. Choose Add -> From question bank ...
      5. Click Page '2' in the paging list.
      6. Click 'Show all N' at the bottom of the list.

      Expected result: all questions are shown.

      Acutal result: no questions are shown (because it is remember the page 2 bit)
