Let add database fields dialogues use Bootstrap to look like other pages

      1. As teacher, add a database activity to a course
      2. Add a field of each type to that database. Alternatively, you can download the attached MDL-70384.featureand run the following command to create a database with a field of each type and then edit these fields one by one: 

        php admin/tool/generator/cli/runtestscenario.php --feature="/full/path/to/MDL-70384.feature"

      3. Verify the adding/editing a database field page does look good.

      1. Install the https://moodle.org/plugins/datafield_linkedcheckbox database field under mod/data/fields/linkedcheckbox.
      2. Add a field of the Linked checkbox type.
      3. Verify the adding a database field dialogue does look good.

      The UI elements where you add fields to a database do use HTML tables.
      This proposal rewrites those to use Bootstrap classes so they look like settings tables elsewhere.
