As a user
I want to be able to see the whole structure of a course in the course index
so I can manage it in a better way.

Acceptance criteria:

1. All the section in the course index are listed and expanded first time a user goes into course.
2. The activities are listed.
3. Only sections and activities that are visible for the user will be listed based on defined visibility, capabilities, access restrictions, etc.
