Question bank: add a new 'Modified by' column

      Set up

      1. Create a course (Course 1)
      2. Go to the course question bank
      3. Create 2 True/False questions
      4. Create a new user
      5. Enroll the created user in the course as a teacher
      6. Go to the Course 1 question bank
      7. Edit those two questions

      Actual test

      1. Log in as the site administrator
      2. Navigate the Course 1 question bank
      3. Confirm that the questions in the question bank have today's date for the modified by column
      4. Confirm that the questions in the question bank have created user's name for the modified by column

      Create a new 'Modified by' column (which now has to get the data to show by getting the latest version of this question and seeing who created it and when.)
