Inconsistent/missing titles/breadcrumbs on cohort management pages

      1. Log in as as admin

      System context

      1. Navigate to Users > Accounts > Cohorts in site administration
      2. Confirm breadcrumbs show 

        Accounts > Cohorts

      3. Confirm page title (see browser title and/or tab title bar) is "System cohorts"
      4. Press All cohorts tab
      5. Confirm breadcrumbs show 

        Accounts > Cohorts > All cohorts

      6. Confirm page title is "All cohorts"
      7. Press System cohorts tab
      8. Confirm breadcrumbs show 

        Accounts > Cohorts

      9. Confirm page title is "System cohorts"
      10. Press Upload cohorts tab
      11. Confirm page title is "Upload cohorts"

      Category context

      1. Navigate to Courses > Manage courses and categories in site administration
      2. Press Actions menu > Cohorts for the default course category
      3. Confirm breadcrumbs show 

        Courses > [Category] > Cohorts

      4. Confirm page title is "Cohorts"
      5. Press Upload cohorts tab
      6. Confirm page title is "Upload cohorts"

      Navigating to Users > Accounts > Cohorts in site administration, the four tabs contain inconsistent and/or missing page titles (that is, the title shown in the browser/tab when viewing the page) and breadcrumbs:

      System cohorts

      1. System cohorts
        • Page title: SITENAME: Administration: Users: Accounts: Cohorts
        • Breadcrumbs: Accounts > System cohorts
      2. All cohorts
        • Page title: SITENAME: Administration: Users: Accounts: Cohorts
        • Breadcrumbs: Accounts > All cohorts
      3. Add new cohort
        • Page title: Add new cohort
        • Breadcrumbs: Accounts > Cohorts > Add new cohort
      4. Upload cohorts
        • Page title: EMPTY
        • Breadcrumbs: Accounts > Cohorts > Upload cohorts

      (MDL-73848 appeared to try and resolve some of the inconsistencies, but didn't really achieve that)

      Category cohorts

      1. Cohorts
        • Page title: Cohorts
        • Breadcrumbs: Courses > CATEGORY > Cohorts
      2. Add new cohort
        • Page title: Add new cohort
        • Breadcrumbs: Courses > CATEGORY > Cohorts > Add new cohort
      3. Upload cohorts
        • Title: EMPTY
        • Breadcrumbs: Courses > CATEGORY > Cohorts > Upload cohorts
