Error: "'name' value must be set" when saving activity settings

I get an error every time I save settings in any kind of activity module.  I have tried: Assignment, Forum, Label, Quiz, and URL.  It happens every time.  Despite the error, the settings do save, and the activities work as they should. 

I am using moodle Moodle 4.0.3+ (Build: 20220826), Adaptable theme, Grid format.  This problem started immediately after I upgraded from moodle 3.11 to moodle 4.

After saving, I see: Coding error detected, it must be fixed by a programmer: The 'name' value must be set in other.
