Duplicate activity heading when completing feedback in classic theme

      1. Log in as admin
      2. Create a new course
      3. Add a Feedback activity
      4. Add a question to the activity
      5. Navigate back to feedback page
      6. Press Answer the questions
      7. Confirm you see activity title only once on page
      8. Switch to Classic theme
      9. Repeat steps 5. to 7. and confirm activity title shown only once on page

      When using the classic theme (or what appears to be any that doesn't use secondary navigation) the activity header is duplicated when a user completes the feedback activity

      Appears to be a regression from MDL-71914 (and/or MDL-72413 where that code was subsequently re-factored into a generic page activity layout)
