Qtype_multichoice: Add a new site-wide default for 'Show standard instruction'

      1. On the top navigation bar click on Site administration
      2. Go to Plugins->Question types ->Multiple choice
      3. Verify that 'Show standard instructions' is a YES/NO select menu with default set to 'No' is added.
      4. For further testing
        1. Go to any Question bank 
        2. Click on 'Create a new question ...' button
        3. Choose 'Multiple choice' and click the 'Add' button
        4. On the question editing form, verify the new settings reflect the default settings from the Multiple choice setting page.

      Add a new site-wide default for 'show standard instruction' and set the default to 'No'. Modify the question form to take the default from the side-wide settings.
