During testing of MDLQA-16834, on the current integration_master branch, I discovered a debug error after adding a file as a submission to an assignment.

To replicate the issue:

1. Ensure there is an Assignment activity available that accepts file submissions
2. Log in as a student
3. Add a submission to an assignment
4. Error should appear
