      Description

      Problem:
      In course/grades.php, grade/lib.php and mod/assignment/lib.php it's used the php function empty() which
      returns the same boolean value for '0' and '' (empty string). So in several places you'll see '-' as if the student
      did not answered when in fact he gave an incorrect answer.

      Despite the fact that this won't change the value of the final grade it can give a more perceptive view of results
      by the teacher.

      Solution:
      See the patch (think I got them all it's been tested for quite a long time since 1.5.2)
