firstslotid Error occurs when attempting to shuffle empty question set

      This was previously reported in MDL-76014, but can be replicable in sandbox.moodledemo.net

       

      When you create a quiz and have no questions added, either checking, or unchecking the shuffle checkbox results in a firstslotid error that 'must be fixed by a programmer'

       

       

      Steps to replicate:

      1. Go to sandbox.moodledemo.net
      2. Create a quiz
      3. Click 'Add Question'
      4. Instead of adding anything, check the shuffle checkbox

      Observed: The above screenshotted error

      This occurs in 4.0.4 but is not a selectable version.
