If question statistics are disabled and then re-enabled under question bank plugins, once re-enabled, the related columns in the question bank are displayed as the first columns rather than returning the columns to their original location.

      STEPS TO REPLICATE

      1. Go into any question bank category and observe the order of the columns.
      2. Go to Site administration > Plugins > Question bank plugins > Manage question bank plugins.
      3. Disable Question statistics.
      4. Return to any question bank category and observe that the "Needs checking?", "Facility index", and "Discriminative efficiency" columns are removed.
      5. Return to Site administration > Plugins > Question bank plugins > Manage question bank plugins.
      6. Enable Question statistics.
      7. Return to any question bank category.

      OBSERVED BEHAVIOR:
The "Needs checking?", "Facility index", and "Discriminative efficiency" columns are now the first columns.

      EXPECTED BEHAVIOR:

The "Needs checking?", "Facility index", and "Discriminative efficiency" columns are returned to their original location between "Comments" and "Usage"

      REPLICATION LOCATIONS
      4.0.5 and 4.1.1 (sandbox.moodledemo.net)

      ADDITIONAL NOTES

Purging caches does not resolve issue with the order of the columns.  Incorrect ordering observed across multiple themes.
