Heading hierarchy skips one level

      1. Test on both Boost and Classic
      2. Install the axe DevTools extension to your browser
      3. On Classic, you may see "Heading levels should only increase by one" issues related to block headings (e.g. on the Navigation block). This is out of the scope of this issue.
      User profile
      1. Go to the user profile.
      2. Run the axe DevTools
      3. Confirm that the following issue is not reported:
        • Heading levels should only increase by one
      mod_lesson
      1. Create a course
      2. Create a lesson in the course. Add a name and press "Save and display"
      3. On the first page, you should see "What would you like to do first?"
      4. Run the axe DevTools on the page.
      5. Confirm that you don't see the following issues:
        • Heading levels should only increase by one
        • Select element must have an accessible name
      6. Click Import questions
      7. Run the axe DevTools on the page.
      8. Confirm that you don't see the following issues:
        • Heading levels should only increase by one
      9. Go back and click "Add a content page". Fill out the required fields (Set Jump field in Content 1 to Next page) and save the changes
      10. On the Editing lesson page, rerun axe DevTools.
      11. Confirm that you don't see the following issues:
        • Heading levels should only increase by one
        • Select element must have an accessible name
      12. Click "Add a question page here"
      13. Create a new True/false question.
      14. Fill out the required fields.
      15. On the entry for the True/False question, click the move button (up/down arrow icon button)
      16. You'll be then on the "Moving page: [question name]"
      17. Run the axe DevTools on the page.
      18. Confirm that you don't see the following issues:
        • Heading levels should only increase by one
      19. Click the "Move to page here" link below the content page that you created earlier so it will appear first in the lesson.
      20. Log in as a student
      21. Go to the lesson
      22. You should now be on the content page.
      23. Run the axe DevTools on the page.
      24. Confirm that you don't see the issue about headings should increase by 1.
      25. Go through the lesson.
      26. At the end of the lesson, you should see "Congratulations - end of lesson reached"
      27. Run the axe DevTools on the page.
      28. Confirm that you don't see the issue about headings should increase by 1.
      29. Log in as admin and go to the lesson
      30. Click on the Reports tab
      31. You should be on the Overview report
      32. Run the axe DevTools on the page.
      33. Confirm that you don't see the following issues:
        • Heading levels should only increase by one
        • Select element must have an accessible name
      34. Go to the Detailed statistics report page
      35. Run the axe DevTools on the page.
      36. Confirm that you don't see the following issues:

      This is a mandatory success criterion for accessibility.

      We need it to be fixed for students using screen readers.

      All headers coloured yellow within the image attached should be marked up as <h2> and not <h3>
