When a teacher is providing feedback to a quiz attempt, the teacher does not have the ability to add media (e.g. image, video, audio) when Tiny is the default editor. This does not happen if you have Atto as the default editor.

To recreate:

Set Tiny as your default editor.
Create a quiz in a course with a student and teacher.
Log in as student and attempt quiz.
Log in as teacher and review attempt and provide feedback.
Notice the lack of Media buttons in the editor.
