Epic to capture the related pieces of work to add Multifactor Authentication (MFA) to Moodle LMS.

There will be several pieces of work to this. Broadly:

• The initial work to include https://github.com/catalyst/moodle-tool_mfa into Moodle LMS MDL-78509
• Improving the workflow for users authenticating (1st) MDL-78534
• Improving the workflow for users setting up their factors (2nd)
• Improving the workflow for admins setting up MFA for their site (3rd)
• Plus several other improvement and clean up tasks
