We recently upgraded from 3.9.11 to 4.1.1 (Build: 20230116). Shortly after the upgrade we discovered that many of our scorm activities were being marked as completed after the learner responded to the first question in the activity. We found this problem in the first 10 activities we tested that included quiz questions. The completion is reported even though the scorm report still showed a completion status of incomplete.
We eventually found that we could change the settings to allow unlimited attempts and this stopped happening. The activity was not marked as completed after responding to the first question. However, that is not a practical solution for us because we would be required to open each course and change the settings.
I have turned on debugging for every test and I have confirmed that a completed or passing status is NOT being sent to Moodle.
I have installed a local copy of 3.9.11 and confirmed that the same activities worked correctly on that version. I have also confirmed this problem on the Moodle 3.11 sandbox and on a personal 4.2 instance hosted by Moodle US, so this is not just happening on our instance.
When we first discovered this problem it was a 2004 activity, but we have confirmed it is also happening with our version 1.2 activities.
