Accessibility issues on the Database activity module

      Setup
      1. An accessibility checker like WAVE or axe DevTools browser extension.
      Test
      1. Log in as an editing teacher to a course. Make sure your teacher account has a picture
      2. Create a database activity module.
      3. Click on "Use a preset"
      4. Run the accessibility checker.
      5. Confirm that there are no accessibility errors related to the presets table and any of its contents.
      6. Use any preset and create an entry
      7. Go to the single view to view the entry you created
      8. Run the accessibility checker on the page.
      9. Confirm that there no accesibility issues on the main content of the page, especially the empty link for the user's picture reported in this issue.

      Some accessibility issues related to the pages in the Database activity module have been raised during the 4.2 accessibility audit:

      Database preset
      1. There are two empty table headers (SC 1.3.1 - Level A)
      2. The four radio buttons in table column one are missing form labels (SC 1.3.1 - Level A)
      3. Usability
        • The group of radio buttons are not encased in a fieldset providing additional context to the purpose of the input fields
      Database view page
      1. Empty link text on the user's picture. (SC 1.1.1, SC 2.4.4, SC 4.1.2 - Level A)
