We should make it consistent with other grade reports to show grade item statuses like locked, hidden icons etc. Currently it only show item name as dimmed if it is hidden

One thing should be taken to account - hidden grades included in average calculations. Wonder if we don't include them, perhaps no sense to show hidden grade items on summary page at all since all grades should be hidden.
