This is a bug we have encountered where quiz activity settings are set so that a student must "receive a grade" but also "grade to pass" is set as well. Students that attempt the quiz but do not achieve the "grade to pass" don't have the activity marked as complete. I believe that this functionality is reserved for when activity completion is set to "receive as passing grade" as opposed to "receive a grade".
