Assignment pdf editor comment icon is hidden

      1. Log in as admin
      2. Create a user
      3. Create a course
      4. Enrol user as student on course
      5. Add assignment to course
        • Submission types: File submission
        • Feedback types: Annotate PDF
      6. Log out
      7. Log in as student
      8. Submit a PDF file to the assignment
      9. Log out
      10. Log in as admin
      11. Navigate to assignment submissions
      12. Press Grade for the test user
      13. Press the Comments option from the PDF annotation toolbar
      14. Add a new comment
      15. Hover over the comment
      16. Confirm the comment editing menu icon is present
      17. Confirm editing menu shows below the icon when pressing the icon
      18. Press Add to quick list
      19. Open comment editing menu again
      20. Confirm the delete icon is appropriately sized

      Try to add a comment in the assignment pdf editor, I am unable to see the comment menu icon.

      This is a regression from Moodle 4.1 which looks fine:

      It is missing in both: LTR and RTL modes.
