There is currently no way in the file API to change the content of a file, and the only way this can be done is by deleting and recreating it. If you want to make this process atomic you would have to do both of these things in a transaction.
It would be useful if the file_storage class had methods to do updates, which would handle the transactions etc.
(This was discussed in the Moodle dev chat and dobedobedoh suggested a patch for this may be welcome.)
