Images don't appear in database list view or forum after uploading (new infrastructure?)

      The images for Chicken Tikka Masala and Paling in 't groen are not appearing in http://learn.moodle.net/mod/data/view.php?d=3 (single view is fine).

      This was also seen happening on moodle.org forums recently in a large forum post with a few images. See Julians post https://moodle.org/mod/forum/discuss.php?d=236638#p1028134

      my hunch is its something about the multiple application nodes. multiple posts going to different servers - ?
