Adding 1 file in a folder shows all files as outdated

When you add a new file in a folder in Moodle, it updates the revision number of all the folder files. This means that the app detects them as outdated, showing a refresh button and re-downloading them all if the user clicks to prefetch the whole folder.

We should try to ignore the revision number for folders. That implies modifying the prefetch delegate, mm-file directive and $mmFilepool.
