Support Lesson Appearance options (menu, progress bar, ...)

      This must be tested in a site with the lesson WebServices integrated.

      1. In web as teacher, create two lessons with only content pages. In one of them, enable all the Appearance options (including a media file and a linked activity). In the other, disable them all.
      2. In the app as student, open the lesson with the appearance options enabled. Start/continue an attempt.
      3. Check that you can see the ongoing score on top. Since the lesson only has content pages it will always be 0 points out of 0.
      4. Check that you can see the progress bar at the bottom of the page. Check that it is updated if you advance in the lesson.
      5. Click the top right icon to open the right side menu. Check that you can see, open and prefetch the media file.
      6. Open the right side menu again. Check that you can see the lesson menu (list of pages) and you can navigate using the menu. Please notice that navigating using this menu doesn't update the progress bar and the lesson will not remember the last page you visited.
      7. Using a tablet in landscape, check that the lesson content doesn't fill the whole width of the tablet (because you enabled the slideshow mode). The width should be 640px.
      8. Navigate to the end of the lesson. Check that you can see a button to open the linked activity. Click it and check that the liked activity is opened. Please notice that activities not supported by the app will be opened in browser. Lessons will be opened in browser, the link handler isn't implemented yet.
      9. Now open the other lesson. Check that you DON'T see the ongoing score, the progress bar and the top right icon. 
      10. Using a tablet in landscape, check that the lesson content fills the whole width of the tablet (because slideshow mode is disabled).
      11. Navigate to the end of the lesson and check that you DON'T see a button to open a linked activity.
