New index page for book module

      This issue only needs to be tested in 1 device.

      1. In the app, open a course and then open any book. Check that, instead of viewing the contents, you now see a page with the description, completion and table of contents. Also, there should be a button saying "Start".
      2. Click "Start". Check that the book is opened with the first chapter, and the new page doesn't display description or next/previous activity bar.
      3. Change chapters and then go back. Remember the last chapter you viewed.
      4. In the entry page, check that now the button says "Resume" instead of Start.
      5. Click "Resume". Check that the book is opened by the last chapter you viewed.
      6. Go back to the entry page and now click any chapter in the table of contents. Check that the book is opened by the chapter you clicked.
      7. Now, in our master site, go to MM Dev Testing course > Links to capture > Activities mod/*view.php?id...
      8. Click "Subchapter 1 in book in this course". Check that the "Subchapter 1" is opened in the book.
      9. Now click "Book in this course". Check that the "Subchapter 1" is opened in the book (this is because it was the last chapter you viewed, if you leave the book in another chapter then that's the chapter it should open).
