Show text and media elements (aka. labels) in the course index

      Course index should now link to all visible modules, including labels and text. For such modules that don't have a page, clicking on them will just scroll to them in the course contents.

      Additionally, clicking an activity with a page should also scroll to their position in the course index before opening (although that may happen too quickly to notice, which is fine).

      This should only happen for 4.2+ sites, for 4.1 and before label and text modules will remain hidden from the course index (when in doubt, they should match the UI in the LMS using the web interface).

      Following MDL-77291 in order to display a consistent course index for students and teachers, we should display labels in the app's course index BUT please notice that this should be only for Moodle 4.2 onward (it is not being backported) so it does not make sense to show something different in web and app for previous versions.
