Create $mmCourseDelegate that handles support for content plugins

      We need $mmCourseDelegate for plugins to register themselves as handlers for the course content. In case a plugin is not defined, a core handler should be assigned.

      We need to provide handlers (as addons) for, at least:

      • label
      • outsourced content (has 1 state, displays outsource icon, shows description if any and little info about content not being accessible from the app)

      The others will be dedicated addons (files, URLs, etc...).
